Passport Information

Instructions for Submitting a Passport Application

  • Everyone must appear in person.
  • Children 15 and under must appear with either both parents or legal guardian in order to submit an application. Click here for more information on requirements for children. http://travel.state.gov/passport/get/minors/minors_834.html
  • Children ages 16 & 17 need only one parent to process the application.
  • The above requirements apply to all children for initial and renewal applications.
  • Adult 10-year passports ONLY, up for renewal are to be mailed directly to the passport agency, please visit above state web site for mailing instructions.

You must bring with you:

Completed Application - "STOP" DO NOT SIGN. Oath must be given prior to signing the application.

Proof of American Citizenship* - Certified Birth Certificate, Previous Passport, Naturalization Certificate or Citizenship Certificate. Birth Certificates for children under 16 must show parents' names (even when submitting a previous passport).

* Original documents must go attached to the application and will be returned along with the new passport.

Identification - Valid U. S. driver's license or valid passport (for adults). If child has a student I.D., they should bring it with them to apply.

Two (2) recent identical passport photos, 2" x 2" in size, show a front view, white/off-white background ONLY.

Appropriate Fees - Checks or Money Orders Only (Please do not write in advance) - 2 Separate checks or money orders for each applicant.

Parental Consent - Both parents must appear in person for children under 16. If one parent cannot appear in person, the absent parent must complete and have notarized form DS-3053 which cannot be more than two weeks old. For foreign born children under the age of 18 please contact our office for information on the required documents.

New Rates below are in effect as of July 13, 2010

Age 16 & over (10 year Passport):

$110 fee check/money order made payable to US Department of State
$25 fee check/money order made payable to Plainsboro Township

Under 16 (5 year Passport):

$80 fee check/money order made payable to US Department of State
$25 fee check/money order made payable to Plainsboro Township

Passports take 4 to 6 weeks for normal processing. Expedited services are available for additional fees. (Plainsboro Township does not participate in courier services)
