We are sharing the following from Middlesex County with the hopes that some of our residents who fit the above criteria will complete the survey. The purpose of the needs assessment survey is to gather valuable data on the service needs of older adults (60+), disabled persons (18+), and their caregivers to further assist their needs. It will help identify existing and potential needs and existing resources, as well as unmet needs, under-utilized services, and barriers that may prevent access to available services.

Alternatively, if you would like to bring them to the Recreation & Cultural Center, we can mail the form out for you, you can place it in a self-addressed, sealed envelope and we can post mark and mail it for you. Copies of the survey and blank envelopes are also available in the Novo Nordisk Fitness & Activity room at the Recreation & Cultural Center.
